So I created a plot and a hole this afternoon, quit, came back, couldn't decide whether I hated the hole more or the plot more, deleted it, and started up again. This is still pretty rough but I feel like it's something I can work with.
Not necessarily the first hole of the course, though. Doing a completely different approach than my usual course design strategy this time where instead of laying out a whole course and basically doing all the holes at once in stages (fairway & green layout, bunkers, major sculpting, tees, etc. etc.), I'm going to just go hole-by-hole, not even trying to string things together at first, finding where on the plot I have a good idea and trying to get that hole at least close to done before moving onto the next. Once I have a bunch of holes laid out I'll start to consider routing as I finish up the rest.
