Lately, whenever I change a club and move the shot marker to a spot that crosses water/sand trap, I constantly get a Perfect/Slow shot ending up in the water/sand.
I have even practiced on my own designs with no wind and it just happens. I have tried just taking the shot I am given before selecting a different club and moving the marker - the result, Perfect/Perfect.
Driving me crazy.

You may be thinking too much about your shot about the past slow DSwings, and trying to "groove" the downswing. Grooving it instead of just letting it happen will sometimes give me a slow as well.
It's kind of like a pitcher in baseball who is trying too hard to hit an exact location in the strike zone. In the attempt to be too fine with one area (precise control), they often lose some base mechanics/fundamentals and also can overlook other areas like release point, etc. They will often miss wildly out of the strike zone or "miss" right down the middle.

Thread title is spot on cliffs - it's in your head dude. Bob highlights why it generally happens... over-focus on one element... plagues even the best sportsmen in the world. Go hit the same shot a dozen times across water to prove to yourself what you already know is true - if a dozen is not enough - take 2 dozen - whatever it needs to break the mental barrier

Believe me, I have been trying. Funny thing....I load a new course and the minute i see water I feel it in the pit of my stomach....might be taking a vacay to the more arid courses on tap... LOL
