This one is going to take some explaining...
Let's start with me. So I have a brother and my sister. My brother is married. Here's where it gets confusing. My brother's wife (sister-in-law) has a sister who is also married. They had kids, and then THOSE kids had a baby (for future reference, the baby's name is Ella)
So basically, my sister-in-law's sister is now a grandma. What would baby Ella be to me? A cousin or a great niece-in-law? I'm just curious... want to see if anyone knows the answer.

Just in case she stumbles upon this forum and reads this post? Good idea to cover your bases.
As far as the question goes I see nothing even remotely resembling an actual family relationship between you and your brother's wife's sister's kid's kid. I suppose if your family gatherings are big enough to include Ella on a regular basis and you feel the need to give yourself an unofficial formal title "Uncle <insert whatever name you're using that week here>" seems most appropriate.
At least that's my take on it and I can't guarantee it's correct.

Boffo is correct. You are not related to your sister-in-law's sister. Or Brother. Or their kids. Your brother is their brother-in-law and their kid's uncle. You are not in the picture as a "relative" outside of being your sister-in-law's brother-in-law and the uncle to their kids.

Yep I agree no relation. But my kids have 4 or 5 people who are long time college friends of my wife and I who they call Uncle or Aunt. Try to go with that if you cant land Godfather which based on your description seems unlikely.

I barely know my sister-in-law's siblings names let alone consider them family. I don't know most of my In-laws other than my wife's side, but we just keep in touch with this part of the family since they live about 10 minutes away from me.
