The course I'm working on now has many bunkers with islands of rough in the middle.
I've tried to figure out how to get the rough to show up with no luck.
When I create the island and create a rough surface (using splines) it won't show through the bunker background.
Is there any way to do it with splines or do I need to piece together shapes - leaving the island in the middle?

Yep, bunker is the most powerful surface in game (I'm pretty sure). Plot out where you want your bunker islands and then put sand around them.

Thanks everyone for help. I may go back and do this on two of my bunkers on Island Springs then (both massive waste bunkers) and make sure I use this technique for the new course I'm working on.

A way I've done this: 1. Draw the inner ring of the bunker that outlines the island rough. Set the spline to 2.0 width. Close the spline, but dont fill. Smooth Path. Since it's 2.0 width allow for that extra 1.0 width of sand plus a few inches because after step 3 it will expand a little. 2. Draw the outside of the bunker, close spline but dont fill, smooth path. 3. Draw another spline (this is the fill) inside the outer edge and inside the inner edge. You can follow the edge of the sand. Start on the outer edge, go all the way around the edge. For the last spline place a point right on your first point. Turn 90 degrees to the inner edge, follow that all the way around in the opposite direction until you get to the end, placing your last spline point again on your first inner edge point. Fill spline, but do not smooth path. It may expand, shrinking your island rough.
Make sure your fill ring is right on the sand edge or else it will not sink and sculpt like bunkers normally do.
You could draw the island rough as a rough spline or use a brush first so you have a line to follow.
I do this because splines give a more precise edge.

Strangest coincidence, I was trying to do this last night by putting rough back into a sand trap and it just didn't work. Amazing this just happened to come up.

you can do it with splines but you need to either create 2 splines or loop around and overlap the spline (takes a little bit of adjustment to look right)
